Student Rating of Teaching (SRT) Forms

Multiple forms are available for use in the course evaluation process. The majority of these forms are considered to be Student Rating of Teaching (SRT) compliant. SRT compliant forms include a standard set of questions addressing general aspects of the instructor and course and demographics. Student release (SR) questions are also included on SRT forms. Faculty members required by University policy to evaluate at least one section of each course taught during a given year must use a SRT compliant form for these evaluations.

 

Student Release Questions
Student release (SR) questions are included on all SRT forms. The SR component consists of ten standard questions.
 
 
Early Semester Evaluations
Early semester (or mid-semester) evaluations are typically offered using Form B - Early Semester. Early semester evaluations are intended for instructors only and are not used in decisions regarding tenure or salary. Form B is not SRT compliant.


Customized SRT Forms
Variations on the student evaluation of teaching (SET) forms are available. Customized forms currently available include:
SRT - Applied Music SRT form with six additional questions for Applied Music (MusA) courses.
SRT - Choral Conducting SRT form with three additional questions for choral conducting courses.
SRT - General Biology SRT form with one additional question to provide an overall rating of the course.
SRT - Music Ensembles SRT form with six additional questions for music ensemble courses.
SRT - Nursing Questions SRT formincluding specific questions for Nursing with reference to the WebCT component.
SRT - Physics SRT form including three additional questions and response areas for four instructor-asked questions, to be answered on a scale of 1 through 7.
SRT - SPH SRT form with 33 additional questions for School of Public Health courses. Additional questions include 21 multiple choice/scaled responses and 12 open-ended questions.
SRT - CVM SRT form with 40 additional questions for College of Veterinary Medicine courses. Additional questions include 28 multiple choice/scaled responses and 12 open-ended questions.
Online Learning with SRT Questions (Twin Cities)
 This is the standard form for online courses of the Twin Cities
 
 
Additional Forms
Biology Laboratory Form Customized form for Biology laboratory sections, intended for evaluation of teaching assistants. This form is not SET compliant.
TA Evaluation Initially created for the evaluation of teaching assistants in Computer Science courses, this form is available for any TA evaluations. Note: This form is intended for TA evaluations only and is not SET compliant.
Online Learning Form (Crookston/Duluth)
Includes Standard general SET questions on instructor and course, plus customized questions for online learning. Does not include demographics or SR.
