SRT form

(Twin Cities, Crookston and Rochester)

Student Rating of Teaching (SRT) Form

Every course with a University course number will be rated by the use of student rating forms every time it is offered, except that thesis-only credits, directed or independent study, and internships will not be rated using such forms. For courses with one instructor but multiple components (e.g., lab, lecture, recitation, etc.), departments have the discretion, to evaluate the components separately. For courses with multiple components, each taught by a different instructor, each component should be evaluated separately.


The SRT form includes a standard set of questions addressing general aspects of the instructor, the learner, the course, and demographics.  Student release (SR) questions are also included on the SRT form.

Form B - Early Semester Student Evaluation of Teaching (SET)Early semester (or mid-semester) evaluations are typically offered using Early Semester Student Evaluation of Teaching (SET) Form B. Early semester evaluations are intended for instructors only and are not used in decisions regarding tenure or salary. Form B is not SRT compliant.
Biology Laboratory Form Customized form for Biology laboratory sections, intended for evaluation of teaching assistants. This form is not SRT compliant.
Online Learning Form (Duluth)

Includes SET questions on instructor and course, plus customized questions for online learning. This form is not SRT compliant. Does not include demographics or Student Release questions.

Student Release Questions

Student release (SR) questions are included on the SRT form. The SR component consists of nine standard questions.
